High-Acuity Medical Respite Center
A 50-bed Medical Respite Center will offer short-term recuperative care and on-site health services for Alameda County residents 18 years and older who are experiencing homelessness and have a complex medical condition or need hospice-level care.

100-Units of housing for Alameda County residents 55 years and older to thrive and age comfortably in the community. The service-enriched senior housing community.
Community Health Care Center
LifeLong’s on-site health center will provide integrated medical, behavioral health, geriatric services, recovery counseling, and hospice care. Quality health care will support clients to resolve acute conditions, stabilize chronic conditions, and receive dignified end-of life care.
Homelessness Prevention Program
The Homelessness Prevention Center will provide case management, rental assistance, and housing placements for an estimated 200 unhoused or vulnerably housed residents of the City of Alameda annually.
